Sildenafil Is Now Widely Available in the UK, So Why Do So Many Men Still Get It Wrong?

Sildenafil has been about since the late 1990s. Even so, loads of men are still confused about what it does, who it suits and where to get it safely. Some of that comes down to erectile dysfunction still being an awkward thing to talk about. The rest is down to the internet, which is full to the brim with dodgy advice on the subject

Once the Viagra patent ran out, generic sildenafil got a lot cheaper and a lot easier to find. A GP can prescribe it, a pharmacy can sell it in limited doses, and a regulated online clinic can supply it too. That last route is where most men under 50 tend to end up, mainly because a face-to-face chat with the doctor about something embarrassing isn't high on anyone's list. Fair enough, really. The stigma is real, even if it probably shouldn't be.

What Sildenafil Actually Does

It's a PDE5 inhibitor. That sounds technical, but the job it does is pretty simple. It relaxes the blood vessels in the penis so more blood can flow in. It won't create arousal by itself, though. Sexual stimulation is still needed for it to work, and a surprising number of men miss that the first time, then decide the tablet hasn't worked.

The usual starting dose is 50mg, taken about an hour beforehand. Some men need 100mg. Others are fine on 25mg. It isn't something to just bump up without thinking, because bigger doses bring a bigger chance of side effects. Headaches, a red face, indigestion and that strange blue tint to everything that some men notice for a while. None of it is usually dangerous, but it's worth knowing about before the first go.

Alcohol is the one that really catches people out. A glass of wine with dinner probably won't ruin things. A heavy night followed by a tablet is a pretty reliable way to make sure it doesn't work and to feel rough as well. A huge, heavy meal beforehand has a similar effect, as it slows down how quickly the tablet gets absorbed.

The Safety Side

For most healthy men, sildenafil is safe. There are exceptions, though. Nitrates for a heart condition and sildenafil are a bad mix. Taken together, they can send blood pressure crashing. That isn't just a box-ticking warning. It's a serious medical risk, and it's why any proper prescriber will ask about heart history and other medication before handing over a prescription.

The trouble with the shadier corners of the internet is that they skip all that. A quick form gets filled in, somebody waves it through, and tablets turn up in the post with barely any clinical check at all. That's a world away from a regulated UK service that actually looks at medical history properly. Anyone looking to buy sildenafil online should care far more about how good that prescribing process is than about the price per tablet.

GPhC-registered online pharmacies and CQC-regulated clinics have the same duties as a GP. They aren't all equally thorough, but they're miles better than unregulated overseas sites flogging unlicensed pills at suspiciously cheap prices. The NHS has been pretty clear on that for years.

ED as a Symptom, Not Just a Problem to Fix

This is the bit most men would rather skip. In men under 40, erectile dysfunction can sometimes point to something else going on. Heart problems, diabetes, high blood pressure, depression, anxiety and low testosterone can all show up this way first. Sildenafil deals with the symptom well, but it does nothing for the cause if there is one.

That's no reason to avoid it. It works well for most men and has helped plenty of people. But for a man in his early thirties where this keeps happening rather than cropping up now and then, getting checked over makes more sense than just managing it with tablets forever. A blood test and a quick chat with a doctor is usually all it takes.

Most of the time, the reason turns out to be nothing scary. Stress, too much booze and bad sleep are all really common causes. Still, it's the sort of thing that deserves five minutes of proper attention, not just a workaround and a subscription box landing on the doormat every month.
